THE TEMA Metropolitan Director of the Factories Inspectorate Department, Stephen Ankama-Lomotey, has identified the main cause of last Friday’s explosion at Delta Agro Limited which claimed the lives of two welders and injured one in the soap manufacturing company. He has attributed the explosion to managerial failure.
